ryoumen sukuna (true form) (jujutsu kaisen)

Ryomen Sukuna's true form is first shown on chapter 3 of Jujutsu Kaisen, depicted as having extra arms, extra eyes and a stomach mouth.
Sukuna would only fully realize his true form during the modern era after possessing Fushiguro Megumi's body and transforming during his battle against Kashimo Hajime on chapter 237.

Do not tag when Sukuna is possessing Itadori Yuuji's body, or when the form's key features are missing, specially the extra eyes on the right side of his face. The main distinctions between Ryomen Sukuna's true form and Itadori Yuuji's possession are his extra arms, stomach mouth, plug earrings, and mainly his overall size and hairstyle, Sukuna's hair is wavier than Yuuji's spiked hair.
