She is a parody of Hisui, with a :3 mouth and solid circle eyes, very simple shortpurple hair, one hand doing a v with the other hand either behind her back or amputated. To compensate her being drawn extremely simple, she is animated very well often using rotoscoping. Other Cookie characters use motion tweens or MMD models with canned animations; Fake Hisui is hand-animated. Another common joke is giving her larger breasts than Hisui.
